Please give 24 hour notice of cancellation prior to your scheduled appointment. If you have missed 3 appointments and not given 24 hour notice prior to those appointments, you may not schedule future appointments.

Please call the office for prescription refills.

Prescription refills will be submitted for approval. If you have not been seen in the office in over 1 year, you must schedule an appointment to receive further medications.
